2. Source & History

The opium poppy is an erect annual plant reaching a height of 1.2 meters with flowerers that vary in colour from white to blue. Its latex juice which is obtained from immature seed capsules one to three weeks after flowering contains several alkaloids including morphine, heroin, codeine, noscapine, papaverine, and thebaine.

Recreational use of the drug began in China in the fifteenth century and became more regularly used by the seventeenth century when it was mixed with tobacco for smoking. Because of its addictive side effects the Chinese emperor tried to stop the opium trade an action which led to the two Opium Wars of 1839 and 1858 in which Britain successfully grew opium in India to beat the blockade.

3. Mentals & Generals

Constitutionally opium presents a picture of stupor, mottled purplish flushed face, stertorous breathing (heavy like snoring), eyes red and half closed, sallow wrinkled countenance and marked emaciation. The cheeks are paretic (paralytic) extending to the tongue and jaw with occasional twitching or tremor.
There is general insensitivity, jerking and numbness of all limbs 

Mentally there are two opposing dispositions. The primary state is one of heightened sensitivity especially of hearing, sight, temperature, both mental and physical pain in general as well as rectal, labour and menses pain specifically. Much perspiration accompanies these symptoms together with extreme restlessness. However despite mental anguish and trauma their mind and thought processes remain clear and acute. 

The secondary state develops as a consequence of  continuous opium consumption and/or exhaustion (resulting from the extreme restlessness of the primary state) in which the patient experiences a "blissed out" state of dreamlike, peaceful, euphoric existence when no pain is felt. The mind is confused, somnolent, dull and inactive. There is an overall sense of fear and foreboding all of which conspire to excite a suicidal gloom. They assert continuously that they are fine and resent interference although the opposite is evident.

The doctrine of primary and secondary states has now been discredited. as the order of occurrence has observed to be different with different patients. What is certain is that both states exist for the same drug and may be present in the same patient. Whether the symptom state is primary or secondary is a matter of opinion.
